Reporting Accidents and Incidents

A recent incident at SK102 highlighted that CPA did not have a procedure for reporting accidents or incidents that may occur on CPA trips or at CPA events. Although these occur very infrequently, accidents and incidents that occur must be reported as soon as possible and in detail to the Coordinator and Steering Committee under our liability policy.  The Coordinator or Treasurer will forward these documents to our insurance agents. Fillable .pdf forms for reporting incidents are now online here. The first page contains the complete reporting procedure. All trip and event leaders should be familiar with these forms, where to find them and how to fill them out. It is suggested that a copy, plus a couple of extra witness forms, be kept handy.
